Suffolk Regional Off-Track Betting Corporation Voluntary Self Exclusion Program
We at Suffolk Regional Off-Track Betting Corporation have a social and business responsibility to promote responsible gambling in the New York racing industry. As such we realize that sometimes recreational gambling can become a problem for some people. We promote responsible gambling and support assistance for those who need it. If you believe that you or someone you care about has a gambling problem we can help. Additionally, individuals can participate in the New York State Gaming Commissi
on Statewide Voluntary Exclusion Program. This program is a New York State mandated program to assist individuals who believe they have a gambling problem by allowing them the opportunity to voluntarily request that they be excluded from entrance into any New York State gaming facilities, including casinos, horse racing, off-track betting, ADW, video lottery gaming, and interactive fantasy sports activities in New York State pursuant to sections 5402 and 5403 of Title 9 of the Official Compilation of Codes, Rules and Regulations of the State of New York and New York Racing, Pari-Mutuel Wagering and Breeding Law section 1404(1)(d).
To sign up for the New York State Gaming Commission Statewide Voluntary Exclusion Program an individual must complete the New York State Gaming Commission Statewide Voluntary Exclusion Form. This Form is available at all Suffolk OTB locations, or by clicking here, by contacting Suffolk OTB’s Customer Service Department at (631) 853-1000 ext. 3524 by going to the New York State Gaming Commission Statewide Voluntary Exclusion Program’s webpage by clicking here.
The notarized form, with photograph as per instructions, should be mailed to Suffolk OTB at 425 Oser Avenue, Suite 2, Hauppauge, NY 11788 Attn: Mike Patrovich – Director of Corporate Security. Please be sure to read the form’s instructions. Incomplete forms will not be able to be processed.
